About Me
I am a board-certified Emergency Medicine doctor who lives in Nevada City. I graduated from Notre Dame, University of Iowa School of Medicine in 2001, and from 2009 to present I have worked at Sierra Nevada Memorial Hospital. Over this period I have also worked at Kaiser Roseville Er, Mercy Mount Shasta Er, and provided primary care at several correctional facilities.
My Practice: In Home Medical Care
My medical practice consists of assisted and independent living facility-visits. I see you there so you don't have to go anywere. I do not maintain a typical clinic. The majority of my patients are considered “home-bound”. I also see “non home-bound” independent living patients who are associated with facilities.
My goal is to make things easier for my patients. If I can arrange in-home testing and treatment, I will. If outside testing, treatment or referral is necessary, I will strive to make it as easy and convenient as possible.
My Patients:
My patients will, generally speaking, be those for whom it makes sense to have a doctor closely involved in their care.
My goal is to help my patients and their family's interface with our health care system in a way that makes sense for them, at their stage in life. I place a high value on each patient getting to make his or her own decisions about how much or how little “health care” they want, with a maximum effort placed on helping them maintain their quality of life.
What I Do offer:
● I will perform regular medical evaluations in the comfort of your residence or in my private home-office. There is no waiting room and you will not be subject to exposure to other patients. There will never be more than one patient at my office at one time.
● I will provide my patients and their families with a HIPAA-compliant secure portal through which they may message me directly and access components of their health record.
● I commit to responding in a timely fashion to any questions or urgent needs.
● I can provide urgent appointments, no longer than 72 hours and often much sooner.
● I can perform minor, “urgent care” type procedures, and help coordinate further testing if needed.
● I know most local hospital-based physicians and specialists personally, and will communicate with them closely if my patient needs referral or hospitalization.
● I take my own call, and after hours and on weekends the phone rings to my cell phone.
What I Do Not Offer:
I, like the majority of primary care physicians, do not admit patients to the hospital, nor will I be their doctor if they are admitted to a Skilled Nursing or Acute Rehab facility.. I do communicate closely with the physicians who care for my patients during admission as well as with discharge planning staff. I will follow them closely and will resume care for my patients upon discharge.
I am not a substitute for an Emergency Room. If there is a true emergency then it needs to be addressed in the ER immediately. If an ER visit is necessary, I can often help coordinate that with the ER provider on shift, as I know most of them. If it can wait until business hours then I will respond to messages as soon as I'm able.
My career as an ER doctor makes me especially suited to helping my patients determine what is and what is not an emergency requiring a trip to the ER. My goal is to prevent unnecessary ER visits as much as possible.
Who Pays For It:
There is a monthly fee to be in the practice (please see the formal contract). This fee is not covered by insurance or Medicare. This allows me to provide for services not covered by insurance, and enables me to be responsive to my patients needs. I also bill Medicare for my visits and covered services. Medicare deductibles and remaining fees are usually covered by secondary insurance, but are ultimately the responsibility of the patient, as is true in any practice.
